If you require any vaccinations relating to foreign travel you need to firstly complete the Travel Risk Assessment form below, followed by an appointment with the Practice Nurse to discuss your travel arrangements.

You should complete the Travel Risk Assessment form as early as possible - at least 6 weeks before you travel.
As soon as we receive the completed Travel Risk Assessment form, you will be contacted by the surgery to make a first appointment with the Practice Nurse to discuss the vaccinations you will need.
At your second appointment your vaccinations will be administered to you. This needs to be at least 2 weeks before you travel to allow the vaccines to work.
**Please note, we do not hold any stock of travel vaccines and these will need to be ordered.**
